West Holt Medical Clinic in Atkinson will be providing sports physicals to area students entering grades 7-12, May 20-24th and July 8-12th from 8 a.m. to 5 p.m. each day. This may be 100% covered by your insurance for the annual well child exam or the cost without insurance is $25. To schedule a sports physical, call 402-925-2811. Sports physical forms and urine cups will be provided at the appointment, please bring your immunization records to your appointment.
